This policy explains what information Empower Chronic Pain (Centennial Physical Therapy), 7173 S. Havana St. Suite 100-12, Centennial, CO 80112, collects through empowerchronicpain.com and how we use it.
Information you give us
- Contact form: your name, email address, phone number and anything you choose to tell us about your pain. We use it to reply to you and to schedule care.
- Patient registration form: your name, email address, date of birth, cell phone number, home address and how you heard about us. We use it to set up your patient record before your first visit.
- Email newsletter: if you sign up, your email address, first and last name and (optionally) your birthday are sent to Mailchimp, our email service, so we can send you updates. Every email has an unsubscribe link.
Please do not send detailed medical records through the website forms. If you are already a patient, health information you share during your care is handled under our separate patient privacy practices.
Information collected automatically
- Cookies: our cookie banner lets you accept, reject or customize non-essential cookies. You can change your choice at any time with the cookie settings button at the bottom of every page. We honor the Global Privacy Control (GPC) signal.
- Site statistics: we use Jetpack (by Automattic) to count visits and see which pages are popular. These statistics are summarized and are not used to identify you.
- Spam and security protection: form submissions are checked by Akismet for spam, and our security tools record IP addresses and browser details to block attacks.
- Embedded content: pages may include content from other sites, such as a Google map or Google Fonts. Those sites may receive your IP address and set their own cookies, under their own privacy policies.
Who we share it with
We do not sell or rent your personal information. We share it only with the service providers that run this website and our communications (our web host, WordPress.com/Jetpack, Akismet, Mailchimp and our email delivery service), or when the law requires it.
How long we keep it
Form messages are kept for as long as we need them to respond to you and provide care, and then deleted. Newsletter details are kept until you unsubscribe.
Your choices and rights
You can ask us to see, correct or delete the personal information we hold about you, or to stop using it for marketing. Residents of some states, including California and Colorado, have additional rights, such as opting out of the sale or sharing of personal information. We do not sell or share personal information for targeted advertising.
Children
This website is not directed to children under 13, and we do not knowingly collect their information online.
